2026: Is the nation’s
safety net still safe?

Seven disclosed cases span young children, girls and adults under guardianship. They should not be reduced to one person’s lapse. The harder question is: between reporting, assessment, visits, placement and follow-up, which safeguard failed to close?

The question is not only whether someone entered the system, but whether the system kept seeing them

Entry into a protection system does not mean risk has disappeared. A safety net needs a trace that can be reviewed, handed over and escalated: who received the information, when a visit occurred, what followed a refused visit, who took over after a move, and what the safety plan was before placement. Every renewed signal requires a new assessment.
